Our Medication Review Services
We offer Medication Review services either at our pharmacy or at your residence for your convenience. This personalized consultation allows us to thoroughly assess your medications and answer any questions you may have.
Eligibility Criteria
You may be eligible for the Medication Review program (in-person or at home) if you meet the following criteria:
- You have a valid Ontario Health Insurance Plan (OHIP) card.
- You take three or more prescription medications for a chronic condition.

You have been diagnosed with Type 1 or Type 2 diabetes.

You live in a licensed long-term care home.

Follow-Up Support
In addition to the initial review, we provide ongoing support. If necessary, you may be eligible for one or more follow-up appointments throughout the year to address any changes or concerns regarding your medications.
Medication Review at Home
If you are eligible for the MedsCheck program but cannot visit the pharmacy, we can bring our services to you with our Medication Review at Home program. During the home visit, our pharmacist will:
- Conduct a medicine cabinet review to evaluate all of your prescription and non-prescription medications.

Identify expired or unused drugs and safely remove them for proper disposal at the pharmacy.
